NW Momentum Health Partners is an Accountable Care Organization (ACO). NW Momentum Health Partners ACO does not have any existing joint ventures. Effective January 1st, 2025, NW Momentum Health Partners is participating in the Medicare Shared Savings Program (MSSP).

Mission

Northwest Momentum Health Partners ACO, LLC is a community of healthcare providers committed to transforming clinical practice with the goal of improving quality, reducing expenditures and enhancing the patient and provider experience.

Management Services

To achieve its mission, NW Momentum Health Partners has implemented a management services agreement with PSW to deliver administrative, operational and clinical services.
