Medicare has new models where health care providers who share a common vision of improving patient care can work together more effectively by belonging to an Accountable Care Organization (ACO). These initiatives are for ACOs that are experienced in coordinating care for populations of patients. The goal is to test whether strong financial incentives for ACOs, coupled with tools to support better patient engagement and care management, can improve health outcomes and lower expenditures for Original Medicare fee-for-service (FFS) beneficiaries. Included in the models are strong patient protections to ensure that patients have access to and receive high-quality care.
NW Momentum Health Partners ACO works with various health care providers across the Pacific Northwest who voluntarily participate, as they believe it will help in providing better quality care for patients. All of the health care providers participating have joined us in committing to the needs and preferences of their Medicare beneficiaries and to emphasize the coordination of care. While beneficiaries may feel as though their care is already coordinated, NW Momentum Health Partners ACO will act as a safeguard to ensure care is always coordinated moving forward.
What is an ACO?
Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients. The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors. When an ACO succeeds both in delivering high-quality care and spending health care dollars wisely, it will share in the savings it achieves for the Medicare program. - Help locating all types of healthcare providers. While you may use any provider who accepts Medicare, NW Momentum Health offers a full network of healthcare providers who are experienced in working with the needs of Medicare beneficiaries.
- Coordination of services when you are moving to or from a hospital or skilled nursing facility. We can also help to organize home care following a stay in a hospital or skilled nursing facility.
- Assistance for patients and their caregivers in managing complex medical conditions including your prescription drugs and medical treatments.
- Information on support systems for seniors and their caregivers.

My provider is participating with NWMHP, what does this mean to me?
Medicare beneficiaries who see providers that are participating in a Medicare ACO keep all their Medicare rights, including the right to choose any doctors and providers that accept Medicare. Whether a provider chooses to take part in an ACO or not, their patients with Medicare may continue to see them.
What is an Annual Wellness Visit?
An Annual Wellness Visit is a chance for you to discuss your health, your medications, and your team of doctors with your primary doctor to ensure s/he can coordinate your care. An Annual Wellness Visit isn’t a physical; it’s a distinct visit that can be received once a year, which lets you have a conversation with your doctor about your health. Such visits have been shown to improve health care for beneficiaries.

How do I opt out of data sharing?
